Impact of Reimbursement Strategies for COPD Among Adults with Medicare Coverage

Chronic obstructive pulmonary disease (COPD) is a leading cause of morbidity and mortality worldwide. COPD adversely affects patients’ quality of life and COPD sufferers consume a large amount of financial and patient care resources. Medicare beneficiaries with COPD may dwell in the community or long-term care facilities.  Beneficiaries with COPD are responsible for the second highest amount of annual inpatient days, and were near the top in the average number of prescriptions and total expenditures consumed during their last years of life.  Treatment is often compounded by concomitant conditions such as asthma, Chronic Heart Failure, and diabetes.  Despite various guidelines and available treatments (occupational, physical, and medicinal therapies), few studies have examined the comparative effectiveness of different treatments in this vulnerable population. Research among this population was previously limited to the Medicare Current Beneficiary Survey, (MCBS a 5% sample of Medicare beneficiaries), and health plan specific databases.  Recognizing the impact of COPD and the need for better data, the Centers for Medicare & Medicaid Services (CMS), included COPD in their Chronic Condition Data Warehouse launched in 2005, with additional information now captured through Medicare Part-D (CMS’ prescription plan initiated in 2006). Relationships between therapeutic and other interventions will be reviewed and discussed in terms of implications for emergency department utilization, hospital admissions and readmissions, costs, and other outcomes.
