Diabetes Management in Patients with Government Supported Benefits

Program Description / Statement of Need:

In 2007 it was reported that 7.8% of the US population has diabetes, a leading cause of death that is associated with: Heart disease and stroke, High blood pressure, Blindness, Kidney disease, Neuropathy, and Amputations. The cost of diabetes in 2007 was estimated at $174 billion. Of the different oral antidiabetic agents, the sulfonylureas, meglitinides, GLP-1 analogues, and DPPIV inhibitors all impact insulin secretion in the pancreas yet these agents work differently. As many of the agents are newer, it is important for the clinician to understand the differing mechanisms of action and responses for each agent.

Target Audience:

This activity has been designed to meet the educational needs of pharmacists, and other health care professionals working for state Medicaid programs who are interested in disease management programs for patients with diabetes, particularly in elderly patients, and those with serious mental illness.

Program Objectives:

After completing this activity, the participant should be better able to:

  1. Recognize that greater reductions in morbidity and mortality among older diabetic patients may result from control of all cardiovascular risk factors rather than from tight glycemic control alone.

  2. Describe that even with the Medicare Part D drug benefit, many older adults with diabetes will incur significant out-of-pocket expenses for their medications

  3. Identify the causes of morbidity and mortality in people with serious mental illness

  4. Recognize that cardiovascular disease is the leading cause of death in persons with mental illness and that type 2 diabetes is a contributor to this increased CV risk
